- Nowy-headed oak board, with a painted depiction of the Cheshire Constabulary crest at top-centre. The inscription is in black lettering, apart from the details of the casualty, which are in red lettering.

- According to information kindly supplied by Cheshire Constabulary Museum, the memorial would originally have been erected at the Hoole Police Station in Chester. Having later been moved to Malpas Police Station (which closed in the 1990s), the memorial is now in Northwich Police Station.
