- Nowy headed brass plaque, wall mounted. The crest of the Welsh Guards surmounts the inscription which is written in Roman capitals in relief.

FELL IN ACTION 3RD SEPTEMBER 1917/ NEAR LANGEMARCK. IN THE THIRD BATTLE OF YPRES/ IN THE GREAT WAR. AGED 19 YEARS./ "HE ASKED LIFE OF THEE & THOU GAVEST HIM A LONG LIFE./ EVEN FOR EVER AND EVER".
