- A rectangular white marble tablet mounted onto a nowy headed grey marble backboard. An additional white marble tablet is mounted on the backboard below. The dedications are written in black lettering on both tablets. At the top of the backboard is a metal badge of the Royal Army Medical Corps, a cross sits below each tablet.
