A brass plaque of unknown form.
Inscription
This tablet is dedicated to the memory of Rifleman Harold Cook, who was killed in action August 24, 1918, aged 22 years. He was organist of this church at the time of his enlistment. Also of Sec. lieut. James Salsbury Smith of Darley Dale, who was killed in action about Dec. 12, 1917, aged 19 years, grandson of the late James Salsbury. (Style and line spacing not known.)
