- Triangular-headed tablet mounted onto a multi coloured stone backboard. Incised inscription in gold and red lettering Curved laurel garland carved in relief at the top of the tablet. Mosaic Maltese cross above the inscription

- TO THE/ GLORY OF GOD/ AND IN MEMORY OF/ JOHN ADDISON STAINSBY/ LIEUT RAMC/ FOR 25 YEARS SURGEON IN/ THIS PARISH AND DISTRICT/ WHO LOST HIS LIFE WHILE/ SERVING HIS COUNTRY ON THE/ HOSPITAL SHIP GLENART CASTLE/ WHICH WAS SUNK BY TORPEDO/ IN THE BRISTOL CHANNEL ON/ THE 26TH FEBRUARY 1918/ DURING THE GREAT WAR/ THIS TABLET WAS ERECTED/ BY HIS MANY FRIENDS IN/ AFFECTIONATE REMEMBRANCE/ OF HIS UNFAILING KINDNESS/ DEVOTION AND SKILL
