- Triptych with hinged doors, and a triangular pediment. The central panel has a raised latin cross. The names are on the central panel and the inside of the doors. Below that section is a rectangular pediment with the dedication.

- When the triptych was originally unveiled in May 1919, it was reported to bear 65 names in black lettering. On an unknown date since, the memorial has been altered, as it now records 71 names, all of which are in gold lettering.

- Forfar Dispatch 22 May 1919 (page 2): reported the unveiling, mentioning that it recorded 65 casualties, whose names were in black lettering.
