- Polished granite stone, with an incised gold-painted inscription within a circle at the top. Beneath that section is the Greater Manchester Police crest, set within five vertical lines; at the foot of the stone 'The Police Memorial Trust' is in incised gold lettering.

- Unveiled on 14/9/2006, the memorial was vandalised in March 2007. The Police Memorial Trust therefore erected a replacement at the same site, which was reported as being in situ by October 2007. Following the posthumous award to Stephen Oake of the Queen's Gallantry Medal in January 2009, the postnominal 'QGM' was duly added to the stone by The Police Memorial Trust.
