Description
An oblong slate tablet in an oak frame. The plain tablet has an incised and gilded inscription and names in four columns in upright capital Roman lettering. The frame has wide side panels which bear signs of once having borne doors and an inscription in capital letters but the doors have been removed, the hinge rebates made good and the inscription erased.

12 July 2011: The inscription on the frame had been erased and was barely legible. The church lies within the Sandiacre Cloudside Conservation Area. 10 November 1967: Church listed Grade I, Heritage List for England number 1204475.
