Rectangular tablet with names surmounted by smaller tablet with dedicatory inscription and date. The whole framed with a decorative black stone frame and apron, supported with two stone corbels.

2003, January: Replacement memorial commissioned (see ref. 63955). Fate of original, damaged memorial unknown. c2002, November: Memorial damaged beyond repair by demolition work at closed church site. c2002: Chapel closed and new church under construction at new site. Memorial to be relocated to new church, but too large for mounting indoors. Plans discussed to erect the memorial on a sheltered wall outside the new church, near the car park.
