- Nowy-headed plain grey slate tablet with incised inscription in gilded lettering. Coloured family arms at top. Death plaque at lower right corner with medal ribbons at left for the Egypt Medal, Queen's South Africa Medal, the 1914 Star, the British War Medal, Allied Victory Medal, the Coronation Medal 1911and the Khedive's Star.
