- Hand Written Roll of Honour on a pre-printed form, in a glazed wooden frame. On the Roll of Honour the words Roll of Remembrance are in a ribbon at the top, there are two union flags below, between them is a Latin cross in an oval, with 'For God, King and Country' around the oval. The rest of the dedication (except 'Resurgam') are in a ribbon around the two union flags with "Resurgam" below the flags. The names are in two columns, and are numbered-1 to 11 in column 1 and 12 to 19 in column 2
