- Three boards set in a wooden frame with a small dedicatory board above. The names on each board in black lettering (served) or red/blue (died).
- Inscription
- THESE ARE THE MEN OF SLAIDBURN PARISH WHO FOUGHT/ FOR THEIR COUNTRY AND THEIR KING ON LAND AND SEA/ IN THE WAR 1914 - 1918. All these were honoured in their generation and were the glory of their times.
