- Marble tablet set on nowy-headed slate backboard. Inscription on centre panel with nowy-headed border. Cross in relief set on partial column centred above inscription. Inscription framed by two protruding columns with ornamentation in relief. Segmented rolled moulding across top of tablet with incurving moulding on lower edge. Inscription is presented in black capital lettering.

- A report appeared in the Mansfield Advertiser, 23 July 1920: ‘A marble tablet erected by public subscription at Teversal Parish Church was unveiled by Lady Carnarvon, and dedicated by the Revd G.E. Eaton (Rector) on Sunday evening in the presence of a large assembly, including discharged soldiers and the relatives of the men.’
